Unable to enter request number when creating a work order

When creating a new work order from Maintenance Super User, the work request number field in the header section of CreateUpdateWOPG is not available.Just the prompt appears and there is no field to enter and no search icon to select from list of values.

This happened after we applied Patch 16187126  to fix the Invalid Asset Number value issue.
The version of CreateUpdateWOPG.xml that causes the issue is
/oracle/apps/eam/workorder/webui/CreateUpdateWOPG 120.36.12010000.69

Any thoughts?
