What is causing RVTSH-189 Receiving Error?

Infrequently, we receive the error message  "RVTSH-189: Subroutine rvtshiline() returned error" after the Receiving Transaction Processor attempts to process PO Receipts.  I have noticed that the header_interface_id is not populated in the rcv_transactions_interface when this occurs.  Is this caused by some sort of user error, a setup issue, a missing patch, etc.?  Any help is appreciated.  Thanks.
