OLE Image File Attachments Fail after going to a newer tools release

There is a known issue causing a lot of confusion / traffic with OLE media object attachments after upgrading to the newer tools releases (9.1.2.3 or newer).  It is documented in E1: MOBJ: Frequently Asked Questions on OLE Media Object Attachments (Doc ID 1305799.1) as Question 14: Why am I unable to attach a Image OLE Media Object Attachment after Upgrading to 9.1.2.3 or newer?

While attaching image files (examples: gif, tif, bmp, png, img, msp) using the OLE option, on Tools Release 9.1.2.3 or newer, we receive the following error.

"File upload to server failed.  Please contact your administrator"
