WARNING: Heavy swapping observed on system in last 5 mins

The owner of the application reported that the application was down a couple of times during the day. I checked the database and it was up. I checked the alert log and found a warning:

WARNING: Heavy swapping observed on system in last 5 mins.

pct of memory swapped in [3.72%] pct of memory swapped out [0.29%].

Please make sure there is no memory pressure and the SGA and PGA

are configured correctly. Look at DBRM trace file for more details.

Could it be that there are too many databases running on that server and there is not enough RAM?  How should i proceed diagnozing the issue? Can someone point me in the right direction?
