BIP 10g does not generate CSV file

Good afternoon,

We are currently on BIP 10g (10.1.3.4.1).  I have created a new report and can display in most formats: html, excel, etc.  However, I can't make it generate a CSV file.  I turned on the debug option but I don't see anything special. 

When I try to send vie e-mail, I do see an error where it doesn't find the temp file it is supposed to generate.

Am I missing a setup or something else?
