Upgrade 11gR2(11.2.0.3) RAC to 11.2.0.4) RAC on OEL 6.4 64 bit

Hello Gurus,
I have 11gR2(11.2.0.3) RAC database running on OEL 6.4 64 bit. We would like to upgrade this RAC database from 11.2.0.3 to 11.2.0.4 including grid and database. Please let me know the patch number to download and docs to peform this activity.
