Create new user but when save get error 20002

We are using EBS version 12.1.3

I am trying to create a new user to the system.

However, after I created the user and assigned the responsibility, when I try to save, I get the oracle error 20002. ORIG_SYSTEM=NULL ORIG_SYSTEM_ID=NULL. Please see attached image. Please advise. Thank you very much.
