OC 12c U4 - a CDOM is missing from its Server Pool

After successfully creating a two node server pool (with two SPARC T4-2s running Solaris 11) and then deploying 6 LDOMs per CDOM node with OS provisioning on each LDOM, the CDOM has gone missing from the Server Pool.I'm not sure when this happened, but it had to be after the initial LDOM creations.
The missing CDOM and its LDOMs are visible in the All Assets view of Ops Center.
If I look back in the job list I can find all the jobs related to ServerPool creation and LDOM provisioning, but nothing related to the CDOM being deleted or removed from the Pool.
