Getting Error "Duplicate relation value - ebs_customer_code" in t_src_loc_tmpl_err table

After running the Demantra collection process, we get three rows for the same customer with the error "Duplicate relation value - ebs_customer_code" in t_src_loc_tmpl_err table.
Due to this all the SO for this specific customer is not being shown up in the worksheet for the forecasters to review.

Any idea what can be the resolution? I searched this community and there was no hit. Metalink has some hits but I am not able to find the resolution.
