personalization on 'Contact Center' form and opening it differently.

Hi,

I have a query regarding form personalization on 'Contact Center' form. I want to hide encircled parts in contact center form. Also I want to open CC form as shown in attached screenshot "personalized Contact Center_2".

Please suggest form personalization/setup/profiles for this.
