T5220 Server Error FATAL: The HOST Processor has a configuration error, forcing a power-down

 Hi,
I need help regarding our one of the client's server T5220 having issue of Error :-

FATAL: Polling POK Status Timeout
FATAL: The HOST Processor has a configuration error, forcing a power-down

We have already replaced MB and Proactively PDB but error came in new board 
We have already upgraded Firmware at  the latest release but same issue

How can we fix this , any help will be appreciable,
