error in create a new task from 'Case Worker'--->'Case Screen'---> 'Resolution and Wrap Up' ---> 'Cr

Hi,

An error is coming like "<!--StartFragment-->oracle.apps.fnd.framework.OAException: java.sql.SQLException: Initialization failed", when we create a new task from 'Case Worker'--->'Case Screen'---> 'Resolution and Wrap Up' ---> 'Create Task of Type' page.

Error stack is as attached in document here.

Please give any idea about this.
