How to create Dynamic columns and rows report using XML Publisher

 Hi,I have a unique requirement as stated below:

The report should generate dynamic columns and rows.
In the attached document , if you notice that for the instructions id 1,2,3
We have different values in the table.
so for all the distinct values for a specific instruction id , we need to get the output as shown in the attached document.

Please suggest on the approach and if we have a sample report , then it would be grateful.
