LOV of the Service request form is not Fetching Value.

Hi All,

We are facing a issue where LOV of the Service request form is not Fetching Value. 

The Service Request Form has two blocks namely BILL_TO_ADDRESS_BLK and SHIP_TO_ADDRESS_BLK which fetches value based on customer number entered from INCIDENT_TRACKING block.

We have applied a Logical App rule to fetch only unique records for the LOV. But the LOV does not fetch any data.
When the rule is disabled the LOV is working fine. There are 2 input parameters for this LOV query. Customer Number and ORG ID. In both, the values are getting passed on to the query. But the query is not fetching the value.
