Batch Status 'P' but approval status code 'I'

Hello,
We observed some batches posted where the approval status code is 'I' 

We are not able to replicate the issue .. but some how the batches are creating some time ..  from last 3 months we can see more than 10 such batches .. 

we are not able to find trend here .. it happening for interface batches as well as Reversal batches .. 

Please help me here ..
