Contact person's city and state details

 Hi,
Can u please suggest me where I can found the contact person's city and state details in External SR creation form.
I need to pull that columns for my report. I am getting customer's details like city,state (con_city,state) in cs_incidents_audit_b table, but I am not able find the 
particular contact person's city which comes under the last_name column in the SR creation form
