Duplcate Receipts got created

There is a PO for qty 90 for an Item, initially one receipt has been made and a second Receipt also got created(not sure whether this was done with out knowledge) for the same item and qty for the same PO. This was done on two occassions. As per my knowledge Human error is ruled out because Expected Receipt form is visible only once, unless there is Partial Receiving.

Any suggestions please...
