Cannot find PAXURVPS_R120.fmb.

Hello,
I checked the source form of the Project Status Inquiry screen and it says the PAXURVPS_R120.fmx can be found at $PA_TOP/forms/US.  I went there and it is not there.  I went to $AU_TOP/forms/US and there is no fmb either.  Do you guys know where this fmb/fmx is found?  Thank you so much in advance for your help.
