Email approval doesn't update Action and notes

 Hi Gurus,

We are implementing WF approvals for requisition. The approval notifications are sent by the WF as emails. When approver gets the email and clicks on 'Approve' , the response email opens up, here user can provide a Note within the quotes ' '. When the approver enters some text within the quotes and hits send on the response email, Oracle recognizes the approval and proceeds to next approver. How ever the next approver in the chain receives the notification with previous approver's action as 'Pending' and the Notes entered by previous approver are not shown. 

Whats more concerning is, if the email is approved from other mail boxes like gmail or other exchange server (not the client's mail box) , the actions and notes make it to Oracle successfully. When approved from within Oracle, everything looks good too.
