R12.1.3 : Patch 16000686 fails with"Keystore was tampered with, or password was incorrect"

Hi,
Noticed this error while applying Patch 16000686 on one of the middle tier.
ERROR: JarSigner subcommand exited with status 1

JarSigner standard output:
jarsigner error: java.lang.RuntimeException: keystore load: Keystore was tampered with, or password was incorrect

JarSigner error output:
Enter Passphrase for keystore:

 Done Analyzing fndlist.jar : Tue Dec 10 2013 11:12:56


ERROR: The following jars failed to get generated properly.
