R12.1.1 -requisition Approval List could not be generated

 Hello,

When the requisition gets above a certain level (25,000) the Approval list does not generate when trying to submit a PR.    Under 25,000 there are three approvers, and up to that level this works fine.

The fourth approver just does not want to work.  Yet the employee is setup exactly the same way as everyone else.

We are using default workflow and have not created our own.  So, the approves should follow the supervisor hierarchy setup under the Employee screen. I've doubled checked the  following:

1) The employees job title are setup

2) The job title is assigned to a approval group.

3) The approval group is the correct range.
