Updating indexes within a table in Oracle R12

I am trying to figure out how to create or add in index within a table. The "Indexes" form within the table form would not allow me to fill in.  Is there a SQL script that needs to be run in order to update?  Attached is a screenprint of what forms within R12 I am referring to.
