Requisition Approval Hierarchy

Dear All,
i have several requisition approval hierarchy. each hierarchy have 5-6 approver level. one of my hierarchy sometime not working as well.
the flow is, if the document already approved by last approver (level 6) then the document status change to APPROVE, as long as the document has not been approved by the last approver, the document status is IN PROCESS.
in my case, after the document is approved by approver level 4, the status change is APPROVE.

please guide me to solve this issue.
