Inventory Transactions Fail with "Total serial numbers does not match the transaction quantity" Erro

We insert transactions into the mtl_transactions_interface and mtl_serial_numbers_interface from a custom application.  Recently, we have sporadically been errors with the "Total serial numbers does not match the transaction quantity" error message.  The quantity for the transactions is 1 and there is 1 serial number.  When I update the mtl_transactions_interface table with the process_flag = 1, revision = 'A' the transaction will process successfully.  Is there a known bug or setting that is causing this seemingly random error to occur?  Any help is appreciated.  Thanks.
