Survey: Your configuration for Oracle EBS 12.1.3+ with shared application tier

I would like to poll the user community for their configuration with Oracle EBS 12.1.3+. Please reply with answers to following questions (also include if you know some clients are using such configuration)

Number of application tier nodes (web/forms, concurrent tiers) and OS:

Number of Database tier node(s)

Shared application tier file system (OCFS2, NFS...)

How do you brace against single point of failure that shared application tier introduces?
