Forecast Consumption Issue? One item forecast is consumed by other item sales order

 In Forecast Consumption, there are cases where the forecast for one item is consumed by another item.  No Product Families are being used in this scenario.  Does anyone know how this can happen?  Is this possibly due to sales order lines that are entered for one item and then later changed to the other item?
We cannot duplicate this at will.
