An end point stall was detected in the USB device

I am getting an fmadm error with a description of :

 An end point stall was detected in the USB device and the
              corresponding driver may not be able to recover from the errors
              automatically.

I have just recently pached the machine and feel it might be a bug in driver issue. 

this is a T4-4 class machine and i do not recall thius issue the last time i patched this machine.

i patch the machine four time per year.

has this been experienced by anyone else?
