Failed Oracle Clusterware upgrade FROM 11.2.0.3.2 to 11.2.0.4

Hello,
I have a failed Oracle clusterware upgrade and do not know how to get back to square one. I've done this upgrade on 3 other clusters with no issues. Here are the particulars for this case:

3 node oracle RAC cluster running clusterware 11.2.0.3.2 on RHEL linux 5
OCR and voting disk are in ASM
Unzipped 11.2.0.4 clusterware and executed runInstaller
Everything was going along hunky dory.
Got to the part where rootupgrade.sh is run on each node.
rootupgrade.sh failed on the first node. The error message was:
