R12 PO Approvals and missing button in WF notifications

Hello,

We seems to have an issue since we upgraded to R12 in that the PO WF approval is missing the buttons now to Approve or Reject the Approval.  Has anyone else seen this or have any more information on this?  We have not customized the WF at all, using the seeded WF.  And although the WF sends correctly, the buttons are missing now like they were in 11.i.  An assistance is greatly appreciated.
