Issue in comprehending PO statuses

Hi, need some help. Attached is a screen shot of a PO - detail revisions, showing next and last statuses. User doesn't know what happened to the PO and I am unable to comprehend it as well. Attached are the Order Activity Rules. Any tips? Thank you.
