Release 9.1 - P0411 - error at the time of saving voucher after entering GL distribution

Hi, we are on Rel 9.1 and facing an issue intermittently that we get a web exception error at the time of saving voucher after entering GL distribution. Sometimes the voucher gets saved all right, sometimes it doesn't. I am not sure why? Has anybody here seen this error? 
Happy to open an SR but then it may not be of much use as it can't really be replicated. Thank you.
