Accessing Planning Form via Smart View produces a pop-up '15' message

Totally stumped on this and Oracle Support couldn't help either

We have a planning form with 10 columns and 4 rows mix of Formula/Data columns/rows.  The form has no problem on browsers or exported but in Smart view when we access the form a small sub-window pops-up with nothing but '15'

All other planning forms work in Smart View.  We have rebuild from scratch and still have the problem.  We do have some special characters in the alias (i.e. '+')

Excel 2010 , Oracle EPM 11.1.2.2 Smart View Build 11.1.2.2.310 (Build 247)
