How to see the changes in PO/Requisition??

Dear All,
Is it possible to check the changes which is made by the Approver while approving the document??  Suppose user has create the PO/Req and send it for approval for 10 items and there approver made the quantity decreses and made it 8 and then approve it. So how to see the changes that what changes is been done??
