Want to generate BIP report in Smart card size, But getting error

 Hi,
I have created a BIP report For Loyalty, named as Membership Card and whose size is same as Smart Card size(3.37" * 2.12").
I took the Vanilla IO "BIP LOY Member Enrolment" and tried to generate the report, I am getting below error. Want to show Member Number and Member Name in the report.
BI Publisher engine failed to generate report.(SBL-RPT-50524)

But same template if I am trying with normal size(A4 in MS Word) its working fine.
Please find the attachment. Also I am attaching the log file which I have generated after getting the error.
