Workflow notification issue - Failed to resreve funds followed by requisition approved

Hi

Has anyone experienced this below issue:

When submitting a requisition a Workflow notification is produced "failed to reserve funds".  A minute later a workflow notifcation is then produced sponutaneously:  "Requisition approved".  This shouldnt happen and is happening with a user for no apparant reason.

Can anyone advise on this please?
