Application upgrade to 11.5.10.2 failed during patch 3480000

Hi ,
Apps 11.5.10

Upgrading  application to 11.5.10.2. During the patch 3480000 we are getting below error.

ATTENTION: All workers either have failed or are waiting:
          FAILED: file arhobsli.sql on worker  1.
          FAILED: file csdrpind.sql on worker  2.
ATTENTION: Please fix the above failed worker(s) so the manager can continue.

adworker log shows
DROP index CS.SUMMARY_CTX_INDEX force

AD Worker error:
The following ORACLE error:

ORA-03113: end-of-file on communication channel
occurred while executing the SQL statement:
DROP index CS.SUMMARY_CTX_INDEX force

AD Worker error:
The following ORACLE error:
ORA-01041: internal error. hostdef extension doesn't exist

Alert log shows..
Errors in file /t01/oracle/proddb/9.2.0/proddb/9.2.0/admin/PROD_ibmp520b/udump/prod_ora_1151428.trc:
