Patch 16000686 : ORA-01408: such column list already indexed

We are applying patch 16000686_R12.HR_PF.B (16000686) in one of the test instance and getting when applying patch
one of the adworker got failed with the below error adworker.log

TTENTION: All workers either have failed or are waiting:
