The Automatic Task Generation Not Working in new R12 instance

Hi, we are upgrading from 11.5.10.2 to 12.1.3. I setup the steps to automatically generate tasks in R12. We do not use this feature in 11i.  Tasks are not getting created automatically when Service Requests are saved/created after performing all setups. Please refer to the attached screenshot. NOTE that this worked on a prior R12 instance which was overwritten by this instance.
