Suspect: Reject Code - Unspecified/Blank

Hi,
There is a "Suspect Rejection" report created from Subject Area: Suspect whereby "Reject Code" is a mandatory field at "Reject a Lead" page layout (Though this layout is not editable; if I'm correct?) application level by default. Now, I've included this field on Lead page layout level whereby it shows as read-only however there are still few records in rejected status where this field is showing as blank at application level & UNSPECIFIED value in real-time report whereas blank in historical report. 

My query is how can it be blank even at application level when its mandatory at "Reject a Lead" page layout by default? And should this Suspect rejection report be created as a real time or historical report for accurate results?
