ASM Instances with RAC Databases

Hi, I am about to create an ASM Client database and I have some fiddly questions !

Q1. Where do you place the ASM Instance ? Is it on each Database Server that an end user database ( i.e. ASM Client database ) has been created on ? I presume the ASM instance cannot sit on a server independent from the Client Databases ( I am using 11gR2 ASM, though I think this capability for ASM to be on a separate server has just been introduced in 12c )

Q2. Can the end user database ( ASM Client Database ) be a higher version number than the ASM Instance version i.e. could I add a new 11gR2 database to a database server which already has 11gR1 databases and an 11gR1 instance of ASM on it ?
