Forms service does not start

Dear All,

i'm doing an upgrade from R12.0.6 to R12.1.1 using note id:752619.1

actually i'm followin the note id:437878.1 and i faced an issue after starting all services and check the forms services

my issue is: forms services does not start

Note:

OS: windows 2003 server

I followed the note id  397174.1 to fix but i faced an issue at this steps: below is txkcfgOC4JApp log
