ORA-30036: unable to extend segment by 8 in undo tablespace 'UNDOTBS1'

Dear Collegues,

We are observing following errors in alert logs frequently on the pre-production boxes that are on solaris 11 & Oracle 11.2

ORA-00604: error occurred at recursive SQL level 1

ORA-30036: unable to extend segment by 8 in undo tablespace 'UNDOTBS1'

Errors in file /u01/app/oracle/diag/rdbms/scp/SCP/trace/SCP_smon_9680.trc:

ORA-30036: unable to extend segment by 8 in undo tablespace 'UNDOTBS1'

I checked on Feb 11th, the table space utilization is only 2%. We got total 4 x 2GB data files in UNDOTBS1.
