R12.1.3 Req Action History: Req approved by other approver

We are on R12:  the user creates a req via iProcurement.  The approval notifications are sent to the correct approval hierarchy.  Each approver approves VIA EMAIL NOTIFICATION (not logging into the application), until the req is approved.  However, the action history shows the following for each approver: Action:  No Action Taken.  Note:  Requisition has been approved by other approver.  I'll upload a screen shot for reference.

Does anyone have any idea why this is happening?
