Help needed configuring forms 11g r2 for High Availability

Hi,

I've been following this document (maa-edg-frd-129690.pdf) about setting up a forms & reports 11gr2 (11.1.2.2)  in a HA configuration but I'm having some problems getting the forms to work correctly from an OHS installation on a different server to the forms application server.

My setup is essentially this (I've left out webhost2 and apphost2 for clarity as I think if I get the issue sorted for one half of the stack then it should work for the other half):

webhost1 listening on port 7777

apphost1 listening on port 9001

There is no webcache installed on either tier, the OHS instance should be handling the requests directly to the forms on the web logic app server.
