'Change Cost Center, Location and Manager Mgr' Not Working for Custom Responsibility

I created a custom responsibility and attached the Menu 'Manager Self Service' to it to allow me access to Change Cost Center, Location and Manager Mgr. However, the launch screen does not show as opposed to when I am using the seeded responsibility. Please view the attached file for details.
