Help with 1099 Electronic Media report

This is the first time we are printing and submitting 1099 from Oracle (R12.1.3), we were able to successfully print the 1099 in pdf formats and have sent to the vendors.
Now when I ran the "1099 Electronic Media report" to submit to IRS electronically, I do get the output but I do see on the first line along with my company name (which appears on the left side of the report) and somewhere from middle Oracle's name and address appears along with the Zip code. Am I missing something here, is it supposed to appear there?
Also I saw on one of the documents where they are saying to put the columns to 750, I did it on the Concurrent Program definition resulting only the view through the output shows 750 columns, but when I check the file on the server, the file has more than 750 columns. Is it supposed to be like that or do I have to change the number of columns on the report to 750? Please advise.
