Creating a link to an SR form using a Web Link field

I'm trying to use a web link field in a SR form to send an email with a link back to the CRMOD form for the Service Request.  I've created the Web Link field and have the URL value set to the following.  This works but the form displayed doesn't include the Related Information sections.  Does anyone know what I need to add to the URL to get the full detailed form displayed?  Keep in mind that there are size limitations to the length of the overall URL.
