My Address Fields on the Account and Contact Pages are now Look-up windows - Help

In my CRM instance, it appears the Address Fields on the Account and Contact Pages have been changed to Look-up windows and users can no longer type text in the address fields.  The Look-up window goes to nothing.  Help!  We want to change the address fields back to the regular way where the user can type in the address information.  The administrator is not sure how these fields got changed to look-up windows.  The fields are now disabled so user cannot type in the address - only click on the look-up window which displays nothing.

Where in the application can I change the Address field back so a user can enter the address information?  We need to change this so a look-up window is not displayed.
