Cant connect to SERVICE using jdbc:thin string for Ops Center 12c to EMCloudControl 12c integration,

We are trying to setup OEM 12c and Ops Center 12c integration per the document below.

- we CAN download the plugin, deploy the opsCenter infrastructure stack plugin on the OracleEntepriseManager 12c's OMS server, and ADD the opsCenter Infrastructure Stack target successfully (using the OEM System Monitoring Plug-in for Oracle Enterprise Manager Ops Center Guide 12c Release 2 (12.1.0.3.0) Part Number E38529-02 doc)

- we are trying to go into the OPS CENTER administration webpage and 'configure enterprise manager cloud control'

-  at the "Configure Connection" step of the Ops Center Administration page, we have the following info:
