Cost Type Form Not Showing Cost Rollup Options

In Release 12.1, when I go to the Cost Type Define Form (CSTFDCTP), the bottom half of the form with the Rollup Options and Previous Level Rollup Options is not visible.  Any idea why?  BOM Parameters are set up for the inventory organization and I'm able to enter BOMS and routings and do a Supply Chain Cost Rollup.  This happens for both existing inventory organizations and also for new inventory organizations that I set up.  The client is using Standard Costing and the Item Attribute Controls for Costing Enabled and Inventory Asset is set to Org Level.  And I am
